Post Adoption Program

CHS was founded in 1891 as an adoption agency by Dr. and Mrs. J.R. Townsend who believed that children belonged in families, not institutions. As the first licensed adoption agency in the State of California, CHS is committed to caring for children and families and continues to adapt to their changing needs. Although our agency no longer places children in adoptive homes, we honor our history by providing post adoption services to adoptees, birth parents, and adoptive parents who have been involved in a CHS adoption. Last year, CHS provided over 1,600 units of Post Adoption Program services to those involved in a CHS adoption.

If you are a CHS adult adoptee, birth parent, adoptive parent of a minor, or child/grandchild/sibling of a CHS adoptee, and would like to request services, please complete the Service Request Form and submit it to CHS via U.S. Postal Service at:

We are very careful to release information to only those permitted to receive it. In order to confirm your identity, you will need to provide CHS with your notarized signature. If you have already submitted a notarized signature to CHS, it is not necessary to submit another. Return only the Service Request Form and indicate your notarized signature is already on file. If your name has changed since the time of the adoption/relinquishment, you must also provide legal proof of your name change (i.e. marriage certificate).

If you are uncertain of the agency that handled the adoption and/or would like to receive information regarding post adoption services in the State of California, you may contact the California Department of Social Services (CDSS) Adoptions Branch. If you are an adult adoptee, birth parent, or sibling and are interested in contact, the State of California has Mutual Consent Programs.
